Mary and Charles Oshinsky Apartment Program (OAP)
You can expect from this program:
- Direct Support Professionals to come to your home and work with you on desired skills
- Drop-in supports from 2 to 10 hours per week based on your scheduling needs
- In-person or a combination of in-person and virtual supports
- Support to make your own choices
- Assistance, if required, with activities for daily living (ADL’s) which could include bathing, dressing, eating and mobility
- Assistance with Instrumental Activities of Daily Living (IADL’s) which could include grocery shopping, nutritional planning, household chores, money management, decision making and meal preparation
- Assistance with learning, improving and maintaining skills
- Private pay only
Henry and Alice Greenwald Personal Supports
You can expect from this program:
- Direct Support Professionals to come to your home, your family home, or within the community to work with you on desired skills
- Drop-in supports from 11 to 82 hours per week based on your scheduling needs
- In- person or a combination of in-person and virtual supports
- Support to make your own choices
- Assistance, if required, with activities for daily living (ADLs) which could include bathing, dressing, eating and mobility
- Assistance with Instrumental Activities of Daily Living (IADLs) which could include grocery shopping, nutritional planning, household chores, money management, decision making and meal preparation
- Assistance with learning, improving and maintaining skills
- DDA is the primary funding source
Supported Living
You can expect from this program:
- Direct Support Professionals help you live independently when you move into a home that you lease or own.
- Dedicated Direct Support Professionals up to 24/7 with and without overnight supports
- Support to make your own choices
- Assistance, if required, with activities for daily living (ADLs) which could include bathing, dressing, eating and mobility
- Assistance with Instrumental Activities of Daily Living (IADLs) which could include grocery shopping, nutritional planning, household chores, money management, decision making and meal preparation
